Appointment request – consultation before birth

Dear parents-to-be,

You are welcome to make an appointment at our outpatient clinic from around the 28th–30th week of pregnancy.

If your pregnancy is progressing normally, the appointment is scheduled from the 36th week of pregnancy.

Please bring the following documents to your appointment:

• Maternity record (Mutterpass)
• Health insurance card from your health insurance fund
• Referral form (Überweisungsschein) from your gynaecologist
• Any current findings, e.g.:
  • doctors’ or surgical reports, previous prenatal findings
  • pre-existing conditions
  • in the case of gestational diabetes: blood glucose readings
