Discharge management / nursing care transition
Counselling for patients and their relatives is an important component of discharge management, ensuring seamless continuing care after your stay in our hospital.
Discharge management is generally available to all patients – especially those who need further support at home after their hospital stay.
We provide individual advice on all questions relating to continuing care and nursing at home, and in cooperation with you and your relatives we arrange and organise all the necessary help and contacts.
By care transition we mean:
- Support and advice for the patient and relatives during the transition from hospital to home.
- Early networking of all the institutions, people and disciplines required, including in the case of palliative care, to ensure a timely and comprehensive transfer to the home environment.
- Clarification of benefit entitlements (e.g. transitional care, care level (Pflegegrad), etc.)
- Arranging services
- Arranging and organising medical aids through medical supply stores and home-care companies.
